Ciclesonide is a nonhalogenated, glucocorticoid prodrug that is hydrolyzed to the pharmacologically active metabolite des-ciclesonide following administration. Des-ciclesonide has a high affinity for the glucocorticoid receptor and exhibits anti-inflammatory activity.

Absorption: 52% (lung deposition)
Protein binding: ≥99%
Metabolism: Ciclesonide hydrolyzed to active metabolite, des-ciclesonide via esterases in nasal mucosa and lungs; further metabolism via hepatic CYP3A4 and 2D6
Excretion: feces(78%) Duration of action: 24 hrs

1. Hypersensitivity to ciclesonide or any component of the formulation
2. Primary treatment of acute asthma or status asthmaticus
3. Moderate-to-severe bronchiectasis

Excretion in breast milk unknown; use caution

Headache, nasopharyngitis, dizziness, fatigue, dysphonia; Upper respiratory infection, epistaxis, nasal congestion, sinusitis, pharyngolaryngeal pain, hoarseness, pneumonia, paradoxical bronchospasm

Adult and adolescent (12 of age and older):
1. Mild asthma: 160-320 mcg once daily
2. Severe asthma: 320-640 mcg once daily OR 320 mcg twice daily
3. Previous oral corticosteroid use: Initial 320 mcg twice daily; MAX 640 mcg twice daily; with gradual reduction of oral corticosteroid (prednisone no faster than 2.5 mg/day on a weekly basis) after at least 1 week of ciclesonide therapy

Children aged 4-11: 160 mcg (1 dose) once daily
