Protein binding: IV: ~94%.
Metabolism: Hepatic via N-glucuronidation, N-methylation, and CYP2A6.
Excretion: Urine (95%); feces (4%).
Clearance: ~39 L/hour.

Hypersensitivity to dexmedetomidine or any component of the formulation.

>10%: bradycardia, hypertension, hypotension, tachycardia, constipation (6% to 14%), nausea (3% to 11%), agitation (5% to 14%), drowsiness, bradypnea, hypoxia, respiratory depression (37%).

General anesthesia, maintenance (adjunctive agent) (off-label use):
Continuous IV infusion: 0.1 to 0.8 mcg/kg/hour; titrate to desired effect.
Patients in the ICU, sedation (adults) :
Initial: 0.5 to 1 mcg/kg for over 10 minutes.
Maintenance dose: 0.2 to 0.7 mcg/kg/hour to sedation goal or clinical effect.
Surgical sedation:
Initial:
Adults: 0.5 to 1 mcg/kg for over 10 minutes.
Patients with flexible scope intubation (awake): 1 mcg/kg for over 10 minutes.
>65 years old: 0.5 for over 10 minutes.
Maintenance dose: Adults: 0.2 to 1 mcg/kg/hour to sedation goal or clinical effect.
Patients with flexible scope intubation (awake): 0.7 mcg/kg/hr.

Class 1, 2, or 3 obesity (BMI ≥30 kg/m2): IV: Use adjusted body weight for initial weight-based dose calculations, then titrate to clinical effect.

No dosage adjustment necessary.
