Anaphylaxis and other severe immediate hypersensitivity reactions: Treatment of type I allergic reactions
Hypotension or shock: Treatment of hypotension associated with septic shock in adults (to increase mean arterial BP)

Alpha-/Beta- Agonist
Stimulates alpha-, beta1-, and beta2-adrenergic receptors resulting in relaxation of smooth muscle of the bronchial tree, cardiac stimulation, and dilation of skeletal muscle vasculature; small doses can cause vasodilation via beta2-vascular receptors, large doses may produce constriction of skeletal and vascular smooth muscle.

1. Onset: ~5-10 min (peak effect: 20 min)
2. Inhalation: ~1 min (peak effect: 3-5 min)
3. Duration: 0.5-2 hrs
4. Elimination half-life: ∼1 min

1. alpha-adrenergic blocker-induced hypotension
2. Cerebral arteriosclerosis, organic heart disease, narrow-angle glaucoma
3. Shock, difficult labor

Dose-related restlessness, anxiety, tremor, cardiac arrhythmia, palpitations, hypertension, weakness, dizziness, and headache

Anaphylaxis and other severe immediate hypersensitivity reactions:
1. IM: 0.3 or 0.5 mg (use 0.5 mg in patients >50 kg); may repeat every ~5 to 15 minutes
2. IV, intraosseous: Initial 0.05 to 0.1 mg (further diluted in 10 mL of NS) administered over 5 to 10 minutes; may repeat once if patient remains unresponsive to initial dose. If patient develops cardiopulmonary arrest, use higher doses (ie, 1 mg [using the 0.1 mg/mL solution] every 3 to 5 minutes) administered rapidly.
Septic shock and other vasodilatory shock states:
Continuous IV infusion:
1. Weight-based dosing: Initial 0.01 to 0.2 mcg/kg/minute; usual dose range: 0.01 to 0.5 mcg/kg/minute; maximum dose range for refractory shock: 0.5 to 2 mcg/kg/minute
2. Non-weight-based dosing: Initial 1 to 15 mcg/minute; usual dose range: 1 to 40 mcg/minute; maximum dose range for refractory shock: 40 to 160 mcg/minute
