Antibiotic, Fluoroquinolone; Antibiotic, Respiratory Fluoroquinolone
For the treatment of bacterial conjunctivitis, community-acquired pneumonia (including cases caused by multi-drug-resistant strains of S. pneumoniae), nosocomial pneumonia, skin and skin structure infections, complicated urinary tract infections (UTIs), acute pyelonephritis and maxillary sinusitis.
藥理
As the S(-) enantiomer of the fluoroquinolone, inhibits DNA-gyrase in susceptible organisms thereby inhibits relaxation of supercoiled DNA and promotes breakage of DNA strands.

Absorption: Rapid and complete
Distribution: Widely distributed in the body, including blister fluid, skin tissue, macrophages, prostate, and lung tissue.
Excretion: Urine (~87% as unchanged drug, <5% as metabolites); feces (<4%).
Half-life elimination: 6 to 8 hours(Adults)
禁忌症
Hypersensitivity to levofloxacin, any component of the formulation, or other quinolones
懷孕分類
Levofloxacin crosses the placenta and can be detected in the amniotic fluid and cord blood
哺乳分類
Levofloxacin is present in breast milk.
Quinolone antibiotics should be avoided in breastfeeding women if alternative agents are available.
副作用
Nausea, headache, diarrhea, insomnia, dizziness, and constipation.
劑量和給藥方法
Pneumonia: Oral, IV: 750 mg once daily.
Chronic obstructive pulmonary disease, acute exacerbation: 500 mg once daily.
Skin and soft tissue infection: 750 mg once daily in combination with other appropriate agents.
Urinary tract infection:
1.Cystitis, acute uncomplicated or acute simple cystitis: 250 mg once daily.
2.Urinary tract infection, complicated: 750 mg once daily for 5 to 7 days
小兒調整劑量
Due to the risk for serious and potentially permanent adverse reactions associated with quinolone antibiotics, levofloxacin should only be used in cases where alternative treatment options cannot be used.
腎功能調整劑量
If usual recommended dose is 500 mg every 24 hours
CrCl 20 to <50 mL/minute: 500 mg initial dose, then 250 mg every 24 hours.
CrC <20 mL/minute: 500 mg initial dose, then 250 mg every 48 hours.
Hemodialysis, intermittent: 500 mg initial dose, then either 250 mg every 48 hours or 125 mg every 24 hours.
