It has bactericidal, amebicidal and trichomonacidal action, active against most obligately anaerobic bacteria, trichomonas vaginalis, Entamoeba histolytic, Giardia lamblia.

In susceptible organisms or cells, metronidazole is reduced by low-redox-potential electron transport proteins (e.g., nitroreductases such as ferredoxin) to unidentified polar product(s) which lack the nitro group. The reduction product(s) appears to be responsible for the cytotoxic and antimicrobial effects of the drug which include disruption of DNA and inhibition of nucleic acid synthesis.

Metronidazole is widely distributed into most body tissues and fluids including bone, bile, saliva, pleural fluid, peritoneal fluid, vaginal secretions, seminal fluid, CSF, and cerebral and hepatic abscesses. Metronidazole is less than 20% bound to plasma proteins. Metronidazole readily crosses the placenta. Metronidazole is distributed into milk in concentrations equal to concurrent plasma concentrations of the drug. Approximately 30–60% of IV dose of metronidazole is metabolized in the liver by hydroxylation, side-chain oxidation, and glucuronide conjugation. The plasma half-life of metronidazole is reported to be 6–8 hours in adults with normal renal and hepatic function.
