Diabetes insipidus: Antidiuretic replacement therapy in the management of central diabetes insipidus and/or temporary polyuria following head trauma in the pituitary region
Hemophilia A: For patients with mild hemophilia A with factor VIII coagulant activity levels >5% of normal to maintain hemostasis during or after surgical procedures; to stop bleeding due to spontaneous or trauma-induced injuries
von Willebrand disease (type 1): For patients with mild to moderate VWD (type 1) with factor VIII coagulant activity levels >5% of normal to maintain hemostasis; to stop bleeding due to spontaneous or trauma-induced injuries

Pituitary Hormone; Vasopressin Analog
Desmopressin acetate is a synthetic analogue of the naturally occurring hormone 8-arginine vasopressin (ADH) and an antidiuretic hormone that affects conservation of water in the kidneys.
Antihemophilic Agent; Hemostatic Agent
Desmopressin acetate is more potent than arginine vasopressin in increasing plasma levels of factor VIII activity in hemophilic individuals and patients with von Willebrand disease type I.

Onset of action: 30 min
Duration: ~6 to 14 hours
Half-life elimination: 2 to 4 hours; in severe renal impairment, extend to ~9 hours
Excretion: Urine (primarily)

1. Hypersensitivity to desmopressin
2. Children under 3 months of age
3. Patients with Type IIB von Willebrand's disease

Excretion in breast milk unknown/use caution

Facial flushing, Blood pressure increased/decreased (I.V.), headache, water retention and hyponatremia, abdominal cramps, nasal congestion, rhinitis, local dermatologic effects following injection, and rash

Diabetes insipidus:
1. Treatment-naive patient: Initial 0.25 to 1 mcg every 12 to 24 hours; may increase to 2 mcg IV if no response to the 1 mcg dose
2. Converting from intranasal desmopressin: Administer one-tenth of the maintenance intranasal dose at the regular intervals
Hemophilia A, von Willebrand disease (type 1):
1. Dosage: 0.3 mcg/kg once (maximum recommended dose: 20 to 30 mcg)
2. Administration: If used for prevention of surgical bleeding, administer 30 to 60 minutes before procedure

Diabetes insipidus:
1. <12 y/o: Adult dosing should not be used in this age groupe (hyponatremia-induced seizures may occur); Some suggest an initial dosage range of 0.1-1 mcg/day in 1 or 2 divided doses; Closely monitor serum sodium
2. Children ≥12 years and Adolescents: 2-4 mcg/day in 2 divided doses or one-tenth(1/10) of the maintenance intranasal dose
Hemophilia A, von Willebrand disease (type 1):
Infants ≥3 months, Children, and Adolescents: 0.3 mcg/kg; if used preoperatively administer 30 minutes before procedure; may repeat dose if needed

1. CrCl 50 mL/minute: There are no dosage adjustments provided in the manufacturer's labeling.
2. CrCl <50 mL/minute: Use is contraindicated according to the manufacturer; however, has been used in acute and chronic renal failure adult patients experiencing uremic bleeding or for prevention of surgical bleeding
