The drug is not protein bound and does not appear to be metabolized; all of a dose is excreted unchanged in the urine. The terminal elimination half-life is 5 to 7 hours.

Common:
Peripheral edema, myalgia, somnolence, fatigue, dizziness, ataxia, nystagmus, weight gain, skin rash, blurred vision, diplopia, mood changes, tremor, hyperactive behavior Significant: 1. CNS and respiratory depression: most cases occur with concomitant use of CNS depressants (especially opioids) in the setting of underlying respiratory impairment, renal impairment, or in older patients <20220218>
2. Neuropsychiatric effects: occurred in pediatric patients (3-12 y/o) with epilepsy, including emotional lability, hostility, changes in behavior, and hyperkinetic muscle activity

Postherpetic neuralgia:
900-1800 mg/day in 3 divided doses. Seizures, focal (partial) onset:
Initial 300 mg 3 times daily; increase dose up to 900-1800 mg/day in 3 divided doses based on response and tolerability (doses up to 2.4 g/day and 3.6 g/day have been tolerated in long-term and short-term clinical studies, respectively)

1. Initial 10-15 mg/kg/day in 3 divided doses, titrate upward over 3 days
2. For children over 5 y/o: Maximum dose 25-35mg/kg/day
