Nicorandil is a nicotinamide ester used for the treatment of angina pectoris, congestive heart failure, and hypertension.

Bioavailability: Oral 75% to 80%.
Onset: Initial Response: Angina, oral: 0.5 to 2 hours.
Duration: Angina, oral: up to 12 hours.
Metabolism: Nicorandil is extensively metabolized via two primary pathways, denitration and oxidation.
Excretion: Renal: 30%, half-life: 40~80 mins.

Previous hypersensitivity to nicorandil, nicotinamide, or nicotinic acid.

Studies in animals have shown evidence of an increased occurrence of fetal damage, the significance of which is considered uncertain in humans.

Infant risk cannot be ruled out.

Vasodilatation, flushing, skin rashes, weight gain, edema, aphthous ulcer of mouth, abdominal pain, anorexia, diarrhea, and nonspecific gastric upset, nausea, vomiting, headache, dizziness, facial palsy, ophthalmoplegia <20210514>

Stable angina: effective oral doses of nicorandil have been 10 to 40 milligrams twice daily.
Variant angina: 5 to 10 milligrams four times daily has been efficacious as add-on therapy in unstable angina patients unresponsive to conventional antianginal therapy.

Dose adjustments are not required.

This half-life prolongation is probably of minimal significance, and dose adjustments in hepatic disease do not appear necessary.
