SmofKabiven is indicated for parenteral nutrition in adults and children over 2 years of age when oral or enteral nutrition is not possible, insufficient, or contraindicated.

SmofKabiven is a parenteral nutrition solution delivering amino acids, glucose, and lipids. The lipid emulsion mimics chylomicrons and contains soybean oil, MCTs, olive oil, and fish oil, supplying essential fatty acids and energy.

本品所含胺基酸、葡萄糖與電解質靜脈注射後直接進入全身循環；脂肪乳劑的清除速率因成分不同而異，其中MCT最速，橄欖油最慢。
Amino acids, glucose, and electrolytes are directly absorbed into systemic circulation. Lipid clearance rates vary: MCTs are fastest, olive oil is slowest, fish oil and LCTs are intermediate.

魚、蛋、大豆、花生過敏、嚴重高血脂、肝功能不全、血液凝集障礙、胺基酸代謝異常、重度腎衰竭（未接受透析）、急性休克、高血糖失控、嬰兒及2歲以下兒童等。
Hypersensitivity to fish, egg, soy, or peanut proteins; severe hyperlipidemia; hepatic failure; coagulation disorders; congenital amino acid metabolism disorders; renal failure without dialysis; uncontrolled hyperglycemia; infants and children under 2 years old.

常見（≥1/100 至 <1/10）：體溫略高。
不常見（≥1/1000 至 <1/100）：食慾下降、噁心、嘔吐、肝酵素升高、寒顫、頭暈。
罕見（≥1/10000 至 <1/1000）：心跳過速、呼吸困難、血壓異常、過敏反應（皮疹、蕁麻疹、紅斑）。

Common (≥1/100 to <1/10): Slight increase in body temperature.
Uncommon (≥1/1000 to <1/100): Nausea, vomiting, elevated liver enzymes, chills, dizziness.
Rare (≥1/10000 to <1/1000): Tachycardia, dyspnea, blood pressure fluctuations, allergic reactions (rash, urticaria).

依據個別病人之體重、營養需求及代謝能力調整。常用劑量範圍為13–31 mL/kg/天，最大為35 mL/kg/天。靜脈滴注，使用中央靜脈給藥，建議輸注時間為14–24小時。
Dose is individualized based on weight, nutritional needs, and metabolic capacity. Typical range: 13–31 mL/kg/day; max: 35 mL/kg/day. Administer via central vein over 14–24 hours.

2-11歲兒童建議最大劑量為35 mL/kg/天；最大輸注速率為2.4 mL/kg/小時。12歲以上可比照成人。
For children 2–11 years: max dose 35 mL/kg/day; infusion rate 2.4 mL/kg/hour. Adolescents (12–18 years): use adult dosing.

重度腎功能不全者（未接受透析）禁用。其他腎功能不全病患應密切監測電解質（特別是磷與鉀），必要時調整劑量。
Contraindicated in severe renal impairment without dialysis. Monitor phosphorus and potassium closely in other cases; adjust dose accordingly.

重度肝功能不全者禁用。肝功能受損者需謹慎使用並監測肝酵素與代謝能力。
