Docetaxel is a semisynthetic antineoplastic analogue of Paclitaxel. It has been approved in treatment of advanced or metastatic breast cancer in patients who have progressed or relapsed during anthracycline-based therapy. Docetaxel has also shown activity against ovarian and non-small cell lung cancer in limited studies.

Potential advantages of docetaxel over paclitaxel include greater anti-tumor activity and lower toxicity potential. No cardiac arrhythmias and myalgias associated with paclitaxel have been observed in patients treated with docetaxel, and hypersensitivity reactions may be less frequent and severe with docetaxel; this latter advantage may be solely due to a different formulation vehicle. Docetaxel is an antimitotic agent. It binds to free tubulin, then promotes the polymerization of tubulin into stable microtubules and inhibits microtubule disassembly, resulting in blockade of cellular mitoticand interphase functions and, consequently, in inhibition of cell division.

Hypersensitivity to docetaxel, paclitaxel, polysorbate 80 (Tween 80), or polysorbate 80-containing preparations. Do not administer docetaxel to patients with neutrophil counts of ＜1500 cells per cubic milliliter.

It is not known whether docetaxel is distributed into milk. Because of the potential for serious adverse reactions to docetaxel in nursing infants, a decision should be made whether to discontinue nursing or the drug, taking into account the importance of the drug to the woman.

neutropenia (dose-limiting), hypersensitivity reactions (up to 60% of unpremedicated patients), asthenia, nausea, vomiting, diarrhea, alopecia, and a sensory neuropathy; mucositis occurs in only 8-10% of patients. A variety of cutaneous reactions and a fluid-retention syndrome have been reported. Cardiac arrhythmias have not been reported with docetaxel.

60-100 mg/m2 IV over 1 hour every 3 weeks. Pre-medication with steroids and diphenhydramine can minimize hypersensitivity and possibly the fluid-retention syndrome.
