Pulmonary embolism: loading dose, 4400 international units/kg IV over 10 minutes (rate, 90 mL/hr), followed by a continuous infusion of 4400 international units/kg/hr IV for 12 hours (rate, 15 mL/hr), flush line at end of infusion; repeat dose as necessary. [Micromedex]

Intra-arterial route: Arterial thrombosis: a)Ductus Arteriosus 1) A loading dose of urokinase 4000 units/kilogram given intravenously over 10 minutes, followed by a maintenance dose of 4000 units/kg/hour infusion for 6 hours has been shown to be successful in restoring arterial pulse with no sequelae; b) Femoral Artery 1) A loading dose of 4000 units/kilogram (kg) over 30 minutes and a maintenance dose of 4000 units/kg/hour in 0.9% sodium chloride were used in one study. Intrapleural route: Pleural effusion a) Study dosage: 20,000 international units via the thoracostomy tube and retained for 2 hours, 3 times daily increased chest tube drainage within 24 hours following instillatio.
