Carbapenem類 Complicated intra-abdominal infections, Complicated urinary tract infections, including pyelonephritis.

Doripenem inactivates multiple essential penicillin-binding proteins (PBPs) resulting in inhibition of cell wall synthesis with subsequent cell death. In E. coli and P. aeruginosa, doripenem binds to PBP 2, which is involved in the maintenance of cell shape, as well as to PBPs 3 and 4.

As with other time-dependent antibiotics, doripenem shows bacteriostatic effects at T>MIC <40% and bactericidal effects at T>MIC>40%. Of note, prolonged infusion time (over 4 hours) was more effective in increasing T>MIC over 40% to up to 81%. T1/2 : ~1 hour; Doripenem is primarily eliminated unchanged by the kidneys. Known serious hypersensitivity to doripenem or other carbapenems (eg, ertapenem, imipenem, meropenem) or any component of the formulation; anaphylactic reactions to beta-lactam antibiotics.

Adult over 18-year-old: I.V.: 500 mg Q8H. Infuse intravenously over at least 1 hour

Dose adjustments according to ClCr (ml/min): - ClCr >50 mL/min: No adjustment necessary; - ClCr 30-50 mL/min: 250 mg Q8H; - ClCr 11-29 mL/min: 250 mg Q12H; Intermittent HD: 250 mg Q24H; if treating infections caused by Pseudomonas aeruginosa, administer 500 mg every 12 hours on day 1, followed by 500 mg Q24H.
