禁忌症
1. Hypersensitivity to heparin or any component of the formulation (unless a life-threatening situation necessitates use and use of an alternative anticoagulant is not possible)
2. Severe thrombocytopenia
3. History of heparin-induced thrombocytopenia (with thrombosis)
4. Uncontrolled active bleeding
5. Not for use when appropriate blood coagulation tests cannot be obtained at appropriate intervals

Due to a better safety profile and ease of administration, the use of low molecular weight heparin (LMWH) is generally preferred over heparin.

Thrombocytopenia
劑量和給藥方法
Intermittent IV bolus: Initial 10,000 units, followed by a continuous bolus of 5,000-10,000 units every 4-6 hours (may be diluted with 50-100 mL normal saline)
Intermittent IV infusion: add 20,000-40,000 units of heparin into 1,000 mL solution
SubQ: Initial 10,000-20,000 units, followed by 8,000-10,000 units every 8 hours or 15,000-20,000 units every 12 hours
