Chemotherapy-induced myelosuppression in nonmyeloid malignancies:
To decrease the incidence of infection (neutropenic fever) in patients with nonmyeloid malignancies receiving myelosuppressive chemotherapy associated with a significant incidence of severe neutropenia with fever
Bone marrow transplantation:
To reduce the duration of neutropenia and neutropenia-related events (eg, neutropenic fever) in patients with nonmyeloid malignancies receiving myeloablative chemotherapy followed by marrow transplantation
Severe chronic neutropenia:
Long-term administration to reduce the incidence and duration of neutropenic complications (eg, fever, infections, oropharyngeal ulcers) in symptomatic patients with congenital, cyclic, or idiopathic neutropenia

Colony Stimulating Factor
Filgrastim is a human granulocyte colony-stimulating factor (G-CSF) which acts on hematopoietic cells by binding to specific cell surface receptors and regulating neutrophil production, progenitor proliferation, and differentiation. It also affects some end-cell functional activation including phagocytic activity, cellular metabolism, and antibody-dependent killing.

Distribution: Vd: 150 mL/kg
Elimination Half Life: approximately 3.5 hr

1. Previous hypersensitivity to myeloid growth factors.
2. Administered within 24 hours, before or after, receiving cytotoxic chemotherapy.

Avoid; because a risk of infant exposure to GCSF exists, caution should be exercised when administering this agent to nursing mothers

Bone pain (Administration of acetaminophen prior to therapy may prevent or reduce the occurrence of bone pain), nausea, vomiting, Influenza-like illness

Avoid use 24 hours before/after cytotoxic chemotherapy.

Chemo-induced (AML/Nonmyeloid): 5 mcg/kg/day (SQ/IV), adjusting up by 5 mcg/kg per cycle if needed. Stop when ANC >10,000/mm³ post-nadir.

Non-chemo Neutropenia: 5 mcg/kg/day (SQ) until ANC ≥500/L for 2 days.

Severe Chronic Neutropenia: Initial 5-6 mcg/kg/day (SQ). Adjust per ANC and clinical response.

Bone Marrow Transplant: 10 mcg/kg/day (IV, 24h post-chemo/marrow).

- ANC >1,000/mm³ for 3 consecutive days: Taper to 5 mcg/kg/day.
- If ANC remains >1,000/mm³ for 3 more consecutive days: Discontinue.
- If ANC decreases to <1,000/mm³: Resume at 5 mcg/kg/day.
- If ANC decreases to <1,000/mm³ during the 5 mcg/kg/day dose: Increase dose to 10 mcg/kg/day and follow the above steps.

Chemotherapy-induced neutropenia (non-myeloid malignancies)

Dose: 5 mcg/kg/day (IV or SC), once daily
Start: 24 hr after chemotherapy
Duration: up to 14 days or until ANC 10,000/mm³
May increase by 5 mcg/kg based on prior response & severity

Bone marrow transplantation (BMT)

Dose: 10 mcg/kg/day (IV infusion)
Start: 24 hr after chemo AND 24 hr after transplant
Adjust by ANC:
- ANC >1,000/mm³for 3 consecutive days: Reduce dose to 5 mcg/kg/day
- If ANC remains >1,000/mm³ for 3 more consecutive days: Discontinue
- If ANC decreases to <1,000/mm³: Resume at 5 mcg/kg/day.
- If ANC decreases to <1,000/mm³ during the 5 mcg/kg/day dose, increase dose to 10 mcg/kg/day and follow the above steps.

Severe chronic neutropenia (SC)

Congenital:
Start 6 mcg/kg/day in 2 divided doses; adjust the dose based on ANC and clinical response.
Median dose: 6 mcg/kg/day.
Idiopathic:
Start 5 mcg/kg/day in 1 or 2 divided doses; adjust the dose based on ANC and clinical response.
Median dose: 1.2 mcg/kg/day.
Cyclic:
Start 5 mcg/kg/day in 1 or 2 divided doses; adjust the dose based on ANC and clinical response.
Median dose: 2.1 mcg/kg/day.

No dosage adjustment necessary.

No dosage adjustment necessary.

Should be stored at 2-8℃. Avoid shaking the solution. Discard any filgrastim solution that has been at room temperature for＞24 hr.
