Antiviral Agent; Neuraminidase Inhibitor:
Peramivir is an antiviral agent (neuraminidase inhibitor), which inhibits neuraminidase activity by binding to the active site of several influenza virus A and B strains.

Severe allergic reaction to any other neuraminidase inhibitor (zanamivir or oseltamivir) or any ingredient in the product.

Patients not requiring hospitalization:
IV: 300 mg(仿單)or 600 mg(UTD) as a single dose; ideally initiate within 2 days of symptom onset.

Patients requiring hospitalization (off-label use):
IV: 600 mg as a single dose, initiate as soon as possible after symptom onset, regardless of duration of symptoms. May extend therapy, depending on severity of illness and response to therapy (eg, 600 mg once daily for 5 days).

Influenza, acute, uncomplicated:

Administer within 2 days of onset of symptoms of influenza.
Infants 6 months and Children:
IV: 12 mg/kg as a single dose; maximum dose: 600 mg/dose.
Adolescents: IV: 600 mg as a single dose.

Influenza, hospitalized, high risk and/or epidemic:

Reserve for patients who cannot tolerate or absorb oseltamivir
29 to 30 days of life: IV: 6~10mg/kg/dose once daily.
31 to 90 days of life : IV: 8~10 mg/kg/dose once daily.
91 to 180 days of life : IV: 10 mg/kg/dose once daily.
181 days of life to <6 years: IV: 10~12 mg/kg/dose once daily; maximum dose: 600 mg/dose.
6 to <18 years: IV: 10~12 mg/kg/dose once daily; maximum dose: 600 mg/dose.
>18 years : IV: 600 mg once daily.

Duration of therapy: Depends on severity of illness and response to therapy; durations of 5 to 10 days have been reported.

1. CrCl 31 to 49 mL/min: 150 mg daily
2. CrCl 10 to 30 mL/min: 100 mg daily
3. CrCl <10 mL/min (not on dialysis or renal replacement therapy): 100 mg on day 1, then 15 mg daily thereafter
4. ESRD on Intermittent hemodialysis: 100 mg on day 1, then 100 mg given 2 hr after each HD session on dialysis days only
