Alkylating Drugs

Analgesics: In alkaline urine, excretion of Aspirin is increased; antacids reduce the absorption of Diflunisal. Antiarrhythmic agents: In alkaline urine, excretion of Flecainide, Mexiletine, and Quinidine is decreased (occasionally resulting in increased plasma concentrations). Antibacterial agents: Antacids reduce the absorption of Ciprofloxacin, Norfloxacin, Ofloxacin, Pivampicillin, Rifampicin, and most tetracyclines. Antiepileptic drugs: Antacids reduce the absorption of Phenytoin. Antifungal agents: Antacids reduce the absorption of Itraconazole and Ketoconazole. Antiplatelet agents: Patients taking Dipyridamole should be advised to avoid concomitant use of antacids. Antimalarial agents: Antacids reduce the absorption of Chloroquine and Hydroxychloroquine

The corrective dose for metabolic acidosis cannot be predicted, and the therapeutic response must be evaluated: generally, 4.8 grams (8 tablets) are administered daily (providing 57 mmol of Na? and HCO??), or higher doses may be required. In cases of severe acidosis, Sodium Bicarbonate should be administered intravenously (IV). Since Sodium Bicarbonate can increase urinary pH, it may also be used as a urinary alkalinizing agent: administer 3 gm dissolved in water every 2 hours until the urinary pH exceeds 7, then maintain alkaline urine with 5–10 gm daily.

Powder for solution (baking soda): Measure dose exactly, mix with adequate amount of water (eg, 1/2 teaspoon powder with 4 ounces of water), allow to dissolve completely prior to administration.

Altered kidney function: No dosage adjustment necessary for any degree of kidney dysfunction. Hemodialysis, intermittent (thrice weekly): No dosage adjustment necessary. Peritoneal dialysis: No dosage adjustment necessary. CRRT: No dosage adjustment necessary. PIRRT (eg, sustained, low-efficiency diafiltration): No dosage adjustment necessary.

There are no dosage adjustments provided in the manufacturer's labeling. Use with caution, especially in clinical states associated with edema and sodium retention.

Store at 15°C to 30°C (59°F to 86°F).
