Treatment (as a single agent) of hepatocellular carcinoma (HCC) in patients who have previously been treated with sorafenib.
Neuroendocrine tumors, locally advanced or metastatic, unresectable:
Treatment (as a single agent) of unresectable, locally advanced or metastatic, well-differentiated pancreatic neuroendocrine tumors (pNETs) or extra-pancreatic neuroendocrine tumors (epNETs) in adults and pediatric patients 12 years of age who have been previously treated.
Renal cell carcinoma, advanced:
First-line treatment (in combination with nivolumab or nivolumab/hyaluronidase) of advanced renal cell carcinoma (RCC).
Treatment (as a single agent) of advanced RCC.

Cabozantinib is a potent inhibitor of proinvasive receptor tyrosine kinases (RTKs), including AXL, FLT-3, KIT, MER, MET, RET, ROS1, TIE-2, TRKB, TYRO3, and VEGFR-1, -2, and -3; induces apoptosis of cancer cells and suppresses tumor growth, metastasis, and angiogenesis

禁忌症
There are no contraindications listed in the manufacturer's US labeling.
Canadian labeling: Hypersensitivity to cabozantinib or any component of the formulation.
懷孕分類
D; Based on its mechanism of action and findings from animal reproduction studies, in utero exposure to cabozantinib may cause fetal harm.
哺乳分類
Due to the potential for serious adverse reactions in the breastfed infant, the manufacturer recommends discontinuing breastfeeding during treatment and for 4 months after the last cabozantinib dose.
副作用
Dermatologic toxicity, Diarrhea, Endocrinopathy, Fistula formation and gastrointestinal perforation, Heart failure, Hemorrhage, Hepatotoxicity, Hypertension, Hypocalcemia, Osteonecrosis of the jaw, Posterior reversible encephalopathy syndrome, Proteinuria and nephrotic syndrome, Thromboembolic events, Wound healing impairment.

60 mg once daily; continue until disease progression or unacceptable toxicity.
Neuroendocrine tumors, locally advanced or metastatic, unresectable:
60 mg once daily; continue until disease progression or unacceptable toxicity.
Renal cell carcinoma, advanced, first-line combination therapy:
40 mg once daily (in combination with nivolumab or nivolumab/hyaluronidase); continue until disease progression or unacceptable toxicity (nivolumab or nivolumab/hyaluronidase may be continued for a maximum of 2 years in patients without disease progression).
Renal cell carcinoma, advanced, single-agent therapy:
60 mg once daily; continue until disease progression or unacceptable toxicity.
Thyroid cancer, differentiated, locally advanced or metastatic:
60 mg once daily; continue until disease progression or unacceptable toxicity.
60 mg once daily; continue until disease progression or unacceptable toxicity.

Kidney impairment prior to treatment initiation:
No dosage adjustment necessary.
Kidney toxicity during treatment:
1. Proteinuria, grade 2 or 3: Withhold cabozantinib until improvement to grade 1 proteinuria, then resume at a reduced dose.
2. Nephrotic syndrome: Permanently discontinue cabozantinib.
No dosage adjustment necessary.
Kidney toxicity during treatment:
1. Proteinuria, grade 2 or 3: Withhold cabozantinib until improvement to grade 1 proteinuria, then resume at a reduced dose.

肝功能調整劑量
Child-Pugh class A: No dosage adjustment is necessary.
Child-Pugh class B:
1. Initial dose 60 mg once daily: Reduce dose to 40 mg once daily.
2. Initial dose 40 mg once daily: Reduce dose to 20 mg once daily.
Child-Pugh class C: Avoid use (has not been studied).
Child-Pugh class B:
1. Initial dose 60 mg once daily: Reduce dose to 40 mg once daily.
2. Initial dose 40 mg once daily: Reduce dose to 20 mg once daily.
Child-Pugh class C: Avoid use (has not been studied).
