適應症
Treatment of adults with symptomatic NYHA class II–III obstructive hypertrophic cardiomyopathy (HCM) to improve functional capacity and symptoms.
藥理
Mavacamten is a selective cardiac myosin inhibitor that reduces excessive actin-myosin cross-bridge formation, thereby decreasing myocardial hypercontractility and improving cardiac function in obstructive HCM.

Absorption :
Tmax approximately 1 hour; bioavailability is not significantly affected by food.
Distribution :
Highly protein bound (>99%).
Metabolism :
Primarily metabolized by CYP2C19 and CYP3A4.
Elimination :
Terminal half-life approximately 6–9 days, but may be substantially prolonged in CYP2C19 poor metabolizers or with CYP450 inhibitors.

禁忌症
Strong CYP2C19 inhibitors; moderate-to-strong CYP2C19 inducers; moderate-to-strong CYP3A4 inducers. Concomitant use may increase mavacamten exposure or reduce its efficacy and increase the risk of heart failure.
懷孕分類
Mavacamten may cause embryo-fetal harm. Confirm absence of pregnancy before initiation. Females of reproductive potential should use effective contraception during treatment and for 4 months after the last dose.
哺乳分類
There are no data on the presence of mavacamten in human milk or its effects on the breastfed infant. Breastfeeding is not recommended during treatment and for 4 months after the last dose.
副作用
Common : dizziness, syncope.
Serious : heart failure due to systolic dysfunction.
Mavacamten may reduce LVEF and cause heart failure, particularly in the presence of drug interactions or other factors that increase mavacamten exposure.

劑量和給藥方法
Recommended starting dose is 5 mg once daily, with or without food. Subsequent doses may be 2.5 mg, 5 mg, 10 mg, or 15 mg once daily, individualized according to LVEF and Valsalva LVOT gradient.
Maximum : 15 mg once daily.
Administration :
1. Echocardiographic assessment of LVEF is required before and during treatment. Do not initiate or up-titrate if LVEF <55%; interrupt treatment if LVEF <50% or if heart failure symptoms/worsening clinical status occur.
2. Swallow capsules whole. May be taken with or without food. Do not crush, chew, or open the capsules.
