Rocuronium bromide is a nondepolarizing neuromuscular blocking agent with intermediate duration and rapid to intermediate onset that is dose-dependent. It mechanism is that It competes for cholinergic receptors at the motor end-plate; however, acetylcholinesterase inhibitors such as neostigmine and edrophonium antagonize this action

Protein binding: approximately 30%; Metabolism- Metabolite: 17-desacetyl-rocuronium; Excretion- Fecal: primary route; Elimination Half Life: adults 27 y to 58 y: 1.4 h +/- 0.4 h

Known hypersensitivity (eg, anaphylaxis) to rocuronium bromide or other neuromuscular blocking agents.

Cardiovascular: Hypertension (0.1% to 2% ), Hypotension (0.1% to 2% ), Tachycardia (less than 1% to 5.3% ); Dermatologic: Injection site pain; Respiratory: Increased pulmonary vascular resistance (24% )

General anesthesia; Adjunct - Rapid sequence intubation: (Premedicated and adequately anesthetized patients) 0.6 to 1.2 mg/kg IV will produce excellent or good intubating conditions in most patients in less than 2 minutes. Muscle paralysis, During surgery or mechanical ventilation: Dosing should be individualized and based on clinical duration following initial dose or prior maintenance dose and not given until recovery of neuromuscular function; a peripheral nerve stimulator should be used to monitor drug effect Maintenance (under opioid/nitrous oxide/oxygen anesthesia) 0.1, 0.15, and 0.2 mg/kg IV given at 25% recovery of first twitch (T1) control (3 twitches of train-of-four) provides a median of 12 minutes (range, 2 to 31 minutes), 17 minutes (range, 6 to 50 minutes), and 24 minutes (range, 7 to 69 minutes) of clinical duration. Continuous infusion, initial 10 to 12 mcg/kg/min IV after early evidence of spontaneous recovery from an intubating dose; initiation after substantial return of neuromuscular function (more than 10% of control T1) may require additional bolus doses to maintain adequate block for surgery.

Diluted infusion solutions should be used within 24 hr of mixing at room temperature; unused portions should be discarded.
